- ASSUMPTION OF RISK. I understand that participation in wrestling, mixed martial arts, grappling, striking, strength and conditioning, and all related activities at American Strength and Combat involves inherent risks of serious bodily injury, including permanent disability, paralysis, and death, as well as damage to personal property. These risks cannot be eliminated regardless of the care taken to avoid them. I knowingly and voluntarily assume all such risks, both known and unknown, and accept full personal responsibility for my participation.
2. RELEASE OF LIABILITY. In consideration of being permitted to train at American Strength and Combat, I, for myself and on behalf of my heirs, assigns, and next of kin, hereby release, waive, and forever discharge American Strength and Combat, its owners, coaches, instructors, employees, volunteers, and landlords from any and all claims, demands, or causes of action arising out of or related to my participation, including claims arising from the ordinary negligence of the released parties, to the fullest extent permitted by California law.
3. INDEMNIFICATION. I agree to indemnify and hold harmless American Strength and Combat from any loss, liability, damage, or cost that may arise from my participation or my presence at the facility.
4. MEDICAL REPRESENTATION AND AUTHORIZATION. I represent that I am physically fit to participate and have no medical condition that would prevent safe participation. I authorize American Strength and Combat to obtain emergency medical treatment on my behalf if needed, at my expense.
5. RULES AND CONDUCT. I agree to follow all gym rules, posted policies, and the instructions of coaches and staff at all times.
6. MEDIA RELEASE (OPTIONAL). I consent to the use of photographs and video taken during training or events for promotional purposes, without compensation.
7. MINOR PARTICIPANTS. If the participant is under 18 years of age, the undersigned parent or legal guardian agrees to all terms above on behalf of the minor and represents that they have legal authority to do so.
